How It Works

Consistent tracking turns vague memories into a clear picture your doctor can act on. Each time a symptom appears, add an entry:

  • Date & symptom: what you felt and when
  • Severity (1-10): a simple self-rated scale so trends are comparable over time
  • Triggers: anything that may have set it off — food, stress, weather, activity
  • Medications: what you took, so you can see what helps
  • Notes: duration, context, or anything else worth remembering

RangeMeaning
1 - 3Mild — noticeable but not disruptive
4 - 6Moderate — interferes with some activities
7 - 10Severe — hard to function; seek care if needed

The trend chart plots the severity of your most recent entries so you can spot patterns at a glance. Use Export PDF to print a clean report (chart plus full history) — in the print dialog choose "Save as PDF" to hand your doctor a summary.
